Privacy Policy

The Federal Health Insurance Portability and Accountability Act addresses the use and disclosure of resident’s medical records and information.  It relates to a resident’s protected health information, marketing or fundraising activities, directories and notification of family and friends concerning resident issues. 

This privacy notice describes how information about residents may be used and disclosed.  Our intent is to comply with the law and make reasonable attempts to limit the unintentional release of protected health information.  Please review it carefully. 

Appropriate health information regarding a resident’s care via consultation, payment, other health care services, assessments and evaluations may be disclosed.  You, or your legal representative, have the right to request how we restrict your protected health information. 

We may use and disclose your information in the following situations:

  • Maintain a directory with names, room numbers, and phone numbers
  • Use communication tools at health care centers, wellness offices and administrative offices.
  • Converse to communicate residents’ needs to staff and providers
  • Disclose general information to family and friends unless otherwise instructed in writing by the resident and/or legal representative
  • Release specific information regarding the resident to individuals identified in the application and on the office record unless instructed otherwise in writing by a legal representative or by the resident.
  • As a nonprofit organization, participate in fund raising activities
  • Use videos, still photographs, or electronic images for advertising, trade or other promotional purposes, including activity and recreational programs
  • Newsletters and newspaper articles. For special uses in advertising, trade or external promotional purposes, such as a brochure or commercial, an authorization shall be obtained from the resident identifying the special use and prior to their use
  • Effectuate reimbursement on the behalf of the resident including but to limited to third party payer, federal, state and local government agencies
